They are well informed and up-to-date and can be reached by phone or e-mail, contact us. Please bring your insurance information with you to the consultation so that we can expedite reimbursement. However, you are responsible for the deductibles, co-pays and office visit fees outlined in your particular insurance plan. Please bring your insurance information with you on the date of your initial consultation. If your insurance company requires a referral number before seeing a specialist, please bring it with you on the day of your visit or your claim may be denied.
